Submit completed forms via email to customerservice@equitybank.com* or via fax to 316-733-5416. Your application will be reviewed and processed within 2 business days.


Summary
  • The Client Setup and Maintenance Form define the services the Company wishes to utilize. Treasury Management Services requested will require additional applications and agreements before implementation of those selected services. A Treasury Management Sales officer will contact the person listed as the Primary Contact to discuss your service needs and begin the process.
  • Please complete the Online Banking Services Agreement and keep a copy for your records.
  • Users defined on the Client Setup form who will need specialized access are defined on the Employee Setup and Maintenance Form. Should you need assistance in completing these enrollment forms, please contact us at customerservice@equitybank.com* or call 888-733-5041.
